PRIM AESTHETICS POLICIES & PATIENT AGREEMENT
Our Promise to Patients
Prim Aesthetics, an Ashmun Street Medical PLLC entity, strives to provide quality care to each patient at each visit. We prefer a customized approach based on individual’s goals and baseline conditions/history. Treatment plans are best devised when both patient and provider are in agreement - with a focus on safety, realistic expectations & timelines, and desired & reasonable outcomes. Out of convenience and courtesy to patients, we offer one free treatment consult per calendar year to facilitate thorough assessment and to formulate a treatment plan.
Refunds | Exchanges
Due to the variables involved in aesthetic, cosmetic, and dermatologic procedures, results and outcomes are not guaranteed. All services and procedures are non-refundable and non-transferrable. You, as the patient, assume the responsibility of fully reading each individual procedure consent form and acknowledge your opportunity to ask any questions of your provider prior to each procedure. For all retail products, including medical grade skin care products, exchanges and refunds will not be given unless the product is found to be defective at the time of purchase.
Follow-Up | Touch-Up Policy
Prim Aesthetics tends to have a conservative approach in regards to dosing of injectable medications, especially for patients who are new to the practice and/or individual provider. At times, a higher dose of injectables is needed to reach the desired outcome. These additional units or doses are charged at regular price. Per provider discretion, 1-3 units of neurotoxin may be administered at no charge for any significant asymmetries. Provider’s preference is for all new or concerned patients to return 2 weeks after injectable treatment for reassessment of the outcome. For patients with concerns regarding dose of toxins (Dysport/Botox), re-evaluation in the office will not be offered until at least 10 days but no more than 20 days have lapsed since initial treatment.
No-Show | Cancellation Policy
We do our best to see patients in a timely manner. We ask that you also make every effort to arrive on time. If you are more than 10 minutes late for a scheduled appointment, we may not be able to honor your appointment and you may be asked to reschedule. Should you need to cancel or reschedule, we require more than 24 hours notice. For a 1st time no-show or late cancellation, you will be given grace. However, for each appointment thereafter, we will apply a $50 fee to the subsequent appointment. We reserve the right to discontinue treatment/ discharge patients from the practice for habitual no-shows.
Electronic Communication
By texting or sending pictures/videos to our phone (906-240-9140) or by messaging our social media accounts, the patient assumes the risk of possible health information compromise, as they are not HIPAA protected means of communication.
COVID-19 Precautions | Mask Removal Risk
We ask that you maintain social distance as able within our office and that you please cancel/reschedule your appointment with us should you be experiencing any symptoms of COVID-19 or been in recent contact with someone who is positive. Due to the nature of the procedures and treatments we offer, you will need to remove your mask, if you're wearing one, once in the exam room with the door closed so the provider may fully assess all aspects your face/neck. By signing this agreement, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk and responsibility that you may be exposed to, or infected by COVID-19, when visiting our office. For your reassurance, the provider, Meghan Hanna FNP, has been vaccinated against COVID-19. .
Disruptive Behavior | Goal Misalignment Policy
While we value all of our patients and make our best effort to provide an agreeable and pleasant treatment experience, we reserve the right to dismiss and refuse care to patients who exhibit disruptive, threatening, or harassing behavior. This includes behavior that is demeaning or belittling to staff. It is in the best interest of both patient and our practice that a good working relationship be maintained. Also in that light, if the provider feels that a patient has goals that would require treatments beyond their scope of practice; patient and provider have differing aesthetic goals; or if care required is beyond the comfort level or capability of the provider, then the patient may be referred elsewhere.
